Patient Forms
You can download the New Patient forms as either a Word document or a PDF document. Once you’ve completed the forms, you can email them to appointment@royaltywellnessspa.com or you can bring them with you to your appointment.
Cancellation- To avoid a charge to your card, please provide Royalty Wellness Spa with a 24 hours notice if cancellation or rescheduling is need.
Reservations- It is recommended to make your appointment as far in advance as possible to ensure availability. A credit card is required to be put on file for all medi-spa reservations.
Breanna Roberson
(Licensed Esthetician)
Breanna is a licensed esthetician that specializes in custom and specialty facials that are tailored to address skin concerns for every individual that comes through the doors of Royalty Wellness Spa! She is also certified in dermaplaning and perfecting her craft in brow waxing, tinting, and lamination.
Breanna graduated from Middle Tennessee State University in 2017 with a B.S. in Organizational Communication and obtained her esthetics degree from The Ivy Institute School of Esthetics in 2020.
