Patient Forms

You can download the New Patient forms as either a Word document or a PDF document. Once you’ve completed the forms, you can email them to appointment@royaltywellnessspa.com or you can bring them with you to your appointment.

Cancellation- To avoid a charge to your card, please provide Royalty Wellness Spa with a 24 hours notice if cancellation or rescheduling is need.

Reservations- It is recommended to make your appointment as far in advance as possible to ensure availability. A credit card is required to be put on file for all medi-spa reservations.
